We offer our A.C.T. Program - Addressing Critical Transitions, which includes outreach crisis response, transitional housing support, employment readiness and job coaching, and mental health and wellness services. The A.C.T. Program is designed to meet individuals where they are and support their journey toward long-term stability.

Yes. All services provided through Healthy H.E.A.R.T.S. are free of charge. We believe everyone deserves safe, dignified support regardless of their financial situation.

Our A.C.T. Program serves single young adult women ages 18-21 who have aged out of foster care, veteran women transitioning back to civilian life, and single parents ages 18-26 experiencing crises. You do not need documentation, insurance, or a referral to connect with us.
